Shipment of your blood sample

  • Download the requisition form and fill it out completely.

  • Enclose the signed requisition form with the samples or send via fax or e-mail to Biofocus.
  • For blood drawing and shipment, please follow carefully the instructions in thade following „Guidelines“ document

  • Prepare the blood samples properly for shipment to us.

Required paperwork / documents for FedEx shipment:
  • International waybill
  • Commercial incoice. For an example click here pdf
  • note: a Certificate of Origin is usually not necessary!
